DISCLAIMER FOR GOOD FAITH ESTIMATE Under the law beginning January 2022, health care providers need to give patients who don’t have insurance or are not using insurance an estimate of the bill for medical items and services. Gals Institute providers will provide you with a financial agreement with self-pay charges for services provided. Please review and make sure you understand the charges for services rendered and our cancellation/no-show policy. By signing the Gals Institute LLC financial agreement, you agree to the financial terms for services provided by a Gals Institute LLC provider. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. Make sure your health care provider gives you a Good Faith Estimate in writing at your first visit with your provider. You can also ask your healthcare provider and any other provider you choose for a Good Faith Estimate before scheduling services. You will be provided the self-pay rate for clinical services provided by a Gals Institute Provider initially at the intake assessment call provided by Gals Institute staff. If you receive a bill at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visitwww.cms.gov/nosurprises or call 262-337-9770.
